The Assisted Living Program... at Pine Crest Village
	is focused on independant living with individualized attention provided as needed and as requested. We know people are living longer, but more of them are living alone. An accident, chronic disease, or progressive decline in functional abilities can easily sideline an individual's ability to live alone. Assisted Living may be desirable when an individual does not require full-time care, but may need help with the activities of daily living, medication supervision, housekeeping, meal preparation, or laundry. The Assisted Living Program at Pine Crest Village offers a special combination of home-like accommodations, supportive services, personalized assistance, and health care designed to respond to those who need help. Pine Crest Village provides each resident up to 28 hours per week of supportive and personal services. These services include a combination of any or all of the following: • Supportive Services Meals, housekeeping in resident apartments, laundry service, and arranging access to medical services. • Personal Services Some assistance with activities of daily living which can include; dressing, meals, bathing, grooming, toileting, transferring and ambulation or mobility. • Health Monitoring Services Assistance with personal medication management and monitoring of vitals on a weekly basis if needed. The determination of a level of care with hours of service per week which a resident receives is, based on a comprehensive assessment by the administrator of Pine Crest Village who, with the resident and family, formulate an individualized written service plan for the resident. To accurately determine what services are necessary for our residents, we develop a Level of Care Assessment that identifies the specific needs of each resident and then matches these with a fee schedule beyond the standard (rate-inclusive) services. The Level of Care Assessment includes assessing the following areas: Personal Care, Mobility, Behavior, Elimination, Medication and Treatment, and other specialized services. A resident receives points in each area that are used to calculate the fee. The final determination lies with Pine Crest Village to set the appropriate level of care for the resident in the vested interest of that resident and in compliance with the State of Wisconsin RCAC regulations. Pine Crest Village is a Certified RCAC (Residential Care Apartment Complex). The resident also has the right to contract for additional services not included in the service agreement from providers of their choice.
